Nominate your provider to join the United Behavioral Health network
Your provider’s participation can help ensure that you and other members in the Chicago area have access to high-quality behavioral health services.
Once the patient completes the form, the provider is contacted by UnitedHealthcare recruitment team to discuss joining the network. There will be no notification to the patient on the outcome, but they may contact their provider’s office to discuss their UnitedHealthcare Choice Plus network status. The provider’s nomination request does not guarantee the provider will join the UnitedHealthcare network.
The final Northwestern University provider network is subject to the completion of recruitment and negotiation discussions between Optum Behavioral Health and the provider, as well as the provider’s acceptance of the negotiated contract terms and fee schedules. All provider networks are subject to change.
